Where to Apply?

Applicants are advised to file their application to the office of the DSWD Community-based Social Workers assigned in their respective provinces. (Please see the list of contact persons below.)

Who may avail of the service?

• Minors who are with persons other than his/her parents.
• Minors who are illegitimate but travelling with their biological father.
• Minor who is travelling alone (13 years old-up)

Note:

The application for travel clearance together with the supporting documents required shall be submitted by the parent or legal guardian or a duly authorized representative at the DSWD Field Offices (Regional) who has jurisdiction over the residence of the minor applicant.

What are the requirements:

A. For First time applicants

1. Duly accomplished application form (click to download form).
2. Authenticated Birth Certificate (PSA) and Passport of the minor if available
3. Notarized affidavit of Parental Consent of both parents/legal guardian (click to download form).Parents who are currently working abroad may secure the ” Affidavit of Parental Consent and Support and Special of Power of Attorney at the Philippine Embassy”
4. Special Power of Attorney (SPA executed by minor’s parents authorizing a relative (4^th degree consanguinity/affinity) to process the travel clearance of the minor
5. Valid ID of parents/legal guardian with SIGNATURE (not expired)
6. Finality of Annulment/Marriage Contract, if parents are married
7. CENOMAR of the mother for a non-marital child
8. Death Certificate, for deceased parents
9. Financial Capability document e.g. COE/Latest Income Tax Return/Bank Statement
10. Two (2) colored passport size picture of the minor with either white, blue & red background (taken w/n the last 6 months) NO SCANNED PICTURE will be accepted.
11. Passport of the travelling companion
12. Medical Certificate (Fit to Travel) if the travelling companion is a Senior Citizen, as per recommendation by the Social Worker
13. Waiver from the parents releasing DSWD from any responsibility/liability in case of untoward incident during the travel of the minor/UM Service Certificate (For travel alone)

B. Renewal

1. Duly accomplished Application Form (click to download form)
2. Photocopy of previous clearance
3. Notarized affidavit of Parental Consent of both parents/legal guardian (click to download form). Parents who are currently working abroad may secure the ” Affidavit of Parental Consent and Support and Special of Power of Attorney at the Philippine Embassy”
4. Valid ID of parents
5. Special Power of Attorney (SPA)
6. Two colored passport size pictures of the minor
7. Photocopy of the passport of the travelling companion
8. Medical Certificate (Fit to Travel) if the travelling companion is a Social Worker, as per recommendation by the Social Worker
9. Waiver from the parents releasing DSWD from any responsibility/liability in case of untoward incident during the travel of the minor/UM Service Certificate (For travel alone)

Note:

Additional documents may be required when found necessary. Submit documents in two copies photocopy and bring the ORIGINAL documents for verification

Maximum of 3 days (for applications with complete documents)
